Best Surgical Result

This award will recognise the consultant plastic surgeon who has created the best surgical facial aesthetic result for a patient. The practitioner may have utilised non-surgical treatments to complement the result. Please note that formatting cannot be copied and pasted from Word documents, so you should only add these when submitting your entry on the website. As well as providing photographic evidence of an excellent aesthetic result, the entry should demonstrate excellent patient care and patient satisfaction with the outcome. The finalists’ before and after images of their patient will be displayed at the Aesthetics Awards, on the Aesthetics Awards website, in the Aesthetics journal and Beyond Beauty consumer magazine, and across social media, so entrants must confirm they have permission to use the images for marketing purposes. Treatment must have been given after January 1 2020. The winner will be selected by the Aesthetics Awards judges.

Matthew Thornton, 48 years of age attended clinic complaining of a very tired and exhausted look. He had lower lid bags for many years, but was never bothered about the appearance. However, as the bags worsened, he eventually reached a point where the bags became very ageing and also gave the patient a very tired and exhausted look. Specialist ophthalmic and oculoplastic consultant Danie Ezra preformed blepharoplasty in a measured way so as not to skeletonise the eye. A “tissue preservation” lower lid blepharoplasty was performed through a subcilliary incision. The lower lid blepharoplasty was predominantly a fat transposition and orbicularis tightening, with minimal skin excision and fat excision. The lower lid was supported with a midface lift, also performed through the subcilliary incision, releasing the orbitomalar and zygomaticofacial ligaments with a subperiosteal dissection fully mobilising the midface to allow for a multiplanar elevation of the cheek. A canthoplasty was also performed. This surgery effectively tightened, lifted and redistributed tissue with very little tissue excised. Matthew was reviewed at one week, 6 weeks and 6 months. The ocular surface was carefully managed, and the patient was delighted with the results.

Miss Elizabeth Hawkes FRCOphth MBBS BSc is a consultant ophthalmic and oculoplastic surgeon, specialising in blepharoplasty surgery, eye surgery and advanced facial aesthetics.

This case highlights a young male with Apert syndrome who presented with a left upper eyelid ptosis having had multiple facial surgical procedures in the past. His vision was unaffected, and therefore, this was classified as a cosmetic issue, rather than a functional problem.

Patients with craniofacial dysgenesis can be stigmatised due to their characteristic facial appearance. The patient wanted to improve the cosmetic appearance of his face and wanted an oculoplastic surgeon who also had cosmetic surgery expertise for the best chance of aesthetic success.

The patient had an excellent surgical result achieving eyelid symmetry and improving the tired appearance of his face. He was delighted with the outcome and it had been a pleasure for me to look after him, after he had placed his trust in me for this complex procedure.

The case highlights how cosmetic eyelid surgery can have a huge impact on quality of life. 

As clinicians, it is important that we are humble with our patients, and fully involve them in every decision making step of the journey.
